O objetivo deste estudo é averiguar a eficácia e segurança deste dispositivo, associado ao uso de Mitomicina C (MMC) subconjuntival, em doentes com GAA.</p><p><strong>Material e Métodos: </strong>Estudo prospetivo, não randomizado, envolvendo 16 olhos de 14 doentes com GAA, não controlados ou com intolerância a tratamento médico, submetidos a implante de <em>XEN 45 Gel Stent</em> juntamente com MMC, com 5 a 12 meses de follow-up (x=7,94m±3,193). Os doentes foram reavaliados no 1º dia, 1ª semana e ao 1º, 3º, 6º e 9º mês de pós-operatório. Foram adquiridos dados em relação à PIO, dimensões da bolha de filtração (avaliada numa escala de 1 a 4), complicações, necessidade de reintervenção e reintrodução de hipotensores. Foi aplicado o teste de <em>Wilcoxon</em> para correlacionar os valores de PIO pre- e pós-operatórios, e o teste de <em>Pearson</em> para determinar a relação entre a redução de PIO e a idade dos doentes. Foi aplicado o teste <em>t de Student</em> para comparação de redução da PIO entre doentes submetidos a cirurgia simples e cirurgia combinada (inserção do XEN Gel Stent e cirurgia de catarata).</p><p><strong>Resultados: </strong>Os valores médios de PIO no pré-operatório foram de 21,56±4.93 mmHg e de 11,62±6.20 mmHg; 12,93±5,95 mmHg, 17,56±5,97 mmHg; 14,07±4.01 mmHg; 14,34±2,77 mmHg e 14,33±4,34 mmHg no 1º dia, 1ª semana, 1º, 3º, 6º e 9º mês, respetivamente. As dimensões da bolha de filtração foram de 3,19; 2,63; 2,56; 2,56; 2,27; 2,11 no 1º dia, 1ª semana, 1º, 3º, 6º e 9º mês, respetivamente. Foram necessárias reintervenções em 7 olhos (3 desobstruções de tubo com laser Nd:YAG; 5 needlings para reabilitação da bolha de filtração; 1 recobrimento conjuntival de tubo extrusado). A redução da PIO foi estatisticamente significativa em comparação aos valores pré-operatórios em todos os pontos avaliados (p<0.01), à exceção do 1º mês de pós operatório. Foi verificada uma correlação inversa, estatisticamente significativa, entre redução de PIO e idade no primeiro dia e aos 6 meses de pós-operatório (r=-0.726; p<0.002 e r=-0.710;p<0.007 respetivamente). Foi possível uma redução do tratamento hipotensor de 88% aos 9 meses de follow-up.</p><p><strong>Conclusões: </strong>O implante do dispositivo <em>XEN 45 Gel Stent</em> em combinação com MMC subconjuntival reduziu de forma significativa a PIO aos 9 meses de follow-up, sem incidência de complicações significativas. Verificou-se a necessidade de recorrer a needling e/ou laser, visando a desobstrução do implante ou revisão de bolha, numa percentagem significativa de doentes.</p><p>Palavras chave: Glaucoma de ângulo aberto, Cirurgia Minimamente Invasiva, XEN 45 Gel Stent.</p><p><strong>Abstract</strong></p><p><strong>Purspose: </strong>The XEN 45 Gel Stent is an innovative device in minimally invasive glaucoma surgery (MIGS) for the treatment of open angle glaucoma (OAG) that is inserted into the anterior chamber through a corneal incision in a conjunctival sparing fashion, allowing for a controlled aqueous humor derivation into the subconjunctival space. The purpose of this study was to investigate its efficacy and security, in association with subconjunctival Mitomycin C (MMC) in the treatment of OAG.</p><p><strong>Material and Methods: </strong>Prospective, nonrandomized clinical study involving 16 eyes of 14 patients with OAG, resistant or intolerant to antiglaucomatous medical treatment, submitted to XEN 45 Gel Stent implantation in association with subconjunctival MMC, with 5 to 12 months of follow-up (x=7,94m± 3,193). Patients were seen on the 1<sup>st</sup> day, 1<sup>st</sup> week, 1<sup>st</sup>, 3<sup>rd</sup>, 6<sup>th</sup> and 9<sup>th</sup> months post-operative. The following variables were studied at all visits: Intraocular pressure (IOP), bubble dimensions (on a subjective scale of 1 to 4), intraoperative and postoperative complications, reinterventions and number antiglaucomatous agents.   Wilcoxon signed rank test was used to correlate pre- and postoperative IOP variation. Pearson correlation was applied to evaluate the connection between IOP reduction and age. Independent t student test was used to compare IOP reduction in patients submitted to sole XEN Gel Stent implantation versus combined surgery with cataract extraction.</p><p><strong>Results:  </strong>Mean preoperative IOP values were 21,65±4,93 mmHg, mean postoperative IOP values were  11,62±6.20 mmHg; 12,93±5,95 mmHg, 17,56±5,97 mmHg; 14,07±4.01 mmHg; 14,34±2,77 mmHg and 14,33±4,34 mmHg on the 1<sup>st</sup> day, 1<sup>st</sup> , 3<sup>rd</sup>, 6<sup>th</sup> and 9<sup>th</sup> months, respectively. Bubble dimensions were 3,19; 2,63; 2,56; 2,56; 2,27; 2,11 on the 1<sup>st</sup> day, 1<sup>st</sup> , 3<sup>rd</sup>, 6<sup>th</sup> and 9<sup>th</sup> months, respectively. 7 eyes were submitted to reinterventions ( Nd:YAG  laser to  restore tube lumen patency,  needlings for filtration bubble rehabilitation and one  autologous conjunctival  transplant to cover a  conjunctival erosion caused by a tube extrusion). The IOP reduction was statistically significant at all timepoints (p<0.01), except for the 1<sup>st</sup> month postoperatively. There was a statistically significant inverse correlation between the IOP reduction and age at the 1<sup>st</sup> day and 6<sup>th</sup> months. (r=-0.726; p<0.002 e r=-0.710; p<0.007 respectively).  It was possible to reduce medical antiglaucomatous treatment by 88% at 9 months.</p><p><strong>Conclusion: </strong>The implantation of the XEN45 Gel Stent in combination with subconjunctival MMC significantly reduced the IOP at 9 months of follow-up, without the incidence of significant complications. There was a need for needling and/or laser to reestablish tube patency and filtration bubble function in a significant number of patients.</p><p>Key words: Open angle glaucoma.
