The authors report the case of an eight-year-old boy who presented to the Pediatric Emergency Department for severe oral mucositis, fever, purulent conjunctivitis, and skin rash. The diagnosis of Mycoplasma pneumoniae-induced rash and mucositis (MIRM) was established. Extrapulmonary complications of Mycoplasma pneumoniae infections are increasingly recognized. Over the years, patients with mucocutaneous manifestations have been misdiagnosed with erythema multiforme spectrum disease. In 2015, MIRM was proposed as a distinct clinical entity with unique pathophysiology, clinical manifestations, and prognosis.
