Risk factors for ischemic stroke patients undergoing intravenous thrombolysis

Introduction: Intravenous thrombolysis has become a standard treatment for patients with ischemic stroke. Functional results after treatment differ.
Objective: To identify the risk factors determining clinical response in ischemic stroke patients undergoing intravenous thrombolysis.
Methods: Systematic review of the literature according to the guidelines established by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Searches were carried out in the electronic databases: Pubmed and CINAHL Complete, with indexing of free access journals of articles from January 2018 to 2023. The search was carried out by four independent reviewers, on October 1, 2023, resulting in a textual corpus of 9 articles with methodological quality.
Results: The risk factors found for patients with ischemic stroke undergoing intravenous thrombolysis were increased age, NIHSS2 score (immediately after thrombolysis), total cholesterol, urea nitrogen, and computed tomography 24 hours after thrombolysis, deep vein thrombosis of the lower limbs, hypertension and cardioembolism. In most cases, IV-rtPA was performed within 3 hours, with hemorrhagic transformation being one of the concerns of IV-rtPA.
Conclusion: This study highlights that late arrival in the time window, mild symptoms and previous stroke are strong predictors of not proceeding with thrombolysis.
