Consumo de medicamentos potencialmente inapropriados e reconciliação de medicamentos em pessoas idosas

Na população idosa, o consumo de medicamentos inapropriados leva ao desenvolvimento de eventos adversos.
Objetivos: Conhecer a dimensão do problema e perceber se a reconciliação de medicamentos é uma solução viável (clínica e economicamente).
Metodologia: Revisão sistemática da literatura, usando o método PICO. Utilizaram-se os descritores “Polimedicação”, “Prescrição Inadequada”, “Reconciliação de Medicamentos”, “Revisão de Uso de Medicamentos” e “Desprescrição”.
Resultados: A existência de prescrições inapropriadas é muito prevalente. A reconciliação de medicamentos é eficaz na sua redução, mas não está provado que é capaz de reduzir os eventos adversos. A falta de envolvimento dos profissionais, a não adesão dos doentes e o custo da implementação são barreiras a ter em conta.
Conclusões: Os programas de reconciliação de medicamentos precisam de ser melhorados para serem clinica e economicamente sustentáveis
