Abstract

Introduction: Insulinoma is a neuroendocrine tumor of the pancreas, with an estimated prevalence of 4 cases per 1 million people/year. In most patients, neuropsychiatric symptoms, secondary to hypoglycemia, are the initial presentation.

Objectives: We reviewed the neuropsychiatric manifestations secondary to insulinoma. Additionally, we report a patient case whose severe psychiatric symptoms proved to be the first clinical manifestation of an insulinoma.

Methods: We performed a systematic search of published literature from 1992 to 2013, through PubMed and reviewed in detail the patient medical files.

Discussion: This case report portrays the difficulty in diagnosing medical conditions that present with clinical symptoms that overlap with neurological/psychiatric syndromes.

Conclusion: The insulinomas may present with neuropsychiatric symptoms, which leads to the complexity and difficulty of an early diagnosis. Thus, we believe that it is critical to conduct a detailed clinical history in order to prevent potential misdiagnosis.
