Retroperitoneal Extra-adrenal Myelolipoma: A Diagnostic Challenge with Pathological Correlation

Myelolipomas are benign tumors composed of adipocites and hematopoyetic cells, presenting in older addults. They commonly occur in the adrenal gland and less frequently in extra-adrenal locations. Their etilogy is unknown, but eventually related to adrenocorticotropic hormonal excess. Most extra-adrenal myelolipomas are detected incedentally on cross-sectional imaging, they have mature fat and radiologically are dificult to diferentiated from other neoplasm with fat, including lipossarcoma.
We present a case of an extradrenal myelolipoma in a perirrenal location and discuss its epidemiology, pathologic features, radiologic features, differential diagnosis, and management.
