Cardiopulmonary arrest caused by a large hiatal hernia

A middle-aged man was admitted to the emergency room with pain, abdominal distension and vomiting. The chest computed tomography showed a gastric volvulus and herniation of the stomach to the intrathoracic space.
During the patient’s transfer to the operating room there was loss of consciousness and cardiopulmonary arrest with the need to start advanced life support and emergent laparotomy.
The authors describe a rare and challenging situation, which was not reported in the literature before, of cardiopulmonary arrest caused by a hiatal hernia, pointing to the importance of being alert to signs/symptoms that may suggest the presence of cardiac compression.
