Resumo

Introdução

A sedação em cuidados intensivos é uma área cuja atuação parece ter implicação no prognóstico do doente. Torna-se pertinente rever a literatura atual e adaptar as orientações à situação clínica e à realidade portuguesa.

 

Métodos

Apresenta-se uma revisão não sistemática cuja revisão bibliográfica foi concluída em dezembro 2020. Foi utilizada a base de dados PubMed com os seguintes termos de pesquisa: sedation, critically ill, intensive care, delirium, circadian dysregulation.

 

Resultados e Discussão

            Qualquer estratégia de sedação deve incidir sobre a utilização de múltiplos fármacos e deve iniciar-se com o controlo da dor do doente. De seguida, titulando de acordo com as necessidades e objetivos, deve instituir-se a sedação através de uma estratégia bem definida de modo a assegurar o nível de sedação mais ligeiro pela menor duração de tempo. 

            A escolha farmacológica e a estratégia sedativa são condicionadas pela situação clínica do doente e pelo seu contexto pelo que se detalham as diferentes especificidades clínicas na escolha farmacológica. 

 

Conclusão

A sedação em UCI deve ser multimodal e regularmente readaptada a cada doente e a cada situação clínica atendendo às recomendações atuais de procurar a mínima profundidade de sedação possível para garantir o conforto e segurança do doente.
