Community Intervention Project: Hyper(A)Tension To Your Heart

Arterial hypertension is a chronic disease, being the leading risk factor for cardiovascular disease causing high mortality worldwide. It is intended to analyze the experience carried out in the Community Nursing internship. A Scoping Review was developed with the question: what are the nursing interventions in empowering of the aged with hypertension? The evidence obtained identified the importance of empowerment in nursing interventions for the aged with hypertension. Based on the Health Planning methodology, and as a structure of community approach to interventions, Betty Neuman's systemic model, the nursing diagnosis was elaborated, which led to the community intervention “Hyper(A)tension to your Heart” with the objective of increasing knowledge regarding arterial hypertension and the potential adoption of healthy behaviors, in the aged with 65 or more years of a social neighborhood. The results point to an average knowledge increase of 22%, demonstrating the positive impact of the project.
