Profilaxia da Endocardite Bacteriana: Estado da Arte

Resumo

Introdução: As recomendações para a profilaxia antibiótica da endocardite têm sido alvo de várias actualizações nos últimos anos com a tendência para simplifica-la e para restringir o número total de doentes elegíveis. Apesar da evidência científica disponível ser escassa e pouco robusta, a mesma foi utilizada para a construção das principais recomendações (da American Heart Association - AHA, European Society of Cardiology - ESC e National Institute for Health and Care Excellence - NICE), que se mostraram contraditórias. Neste estudo pretendemos rever a literatura mais recente relativamente à profilaxia da endocardite bacteriana para promover a correta utilização das recomendações em vigor.

Métodos: Esta revisão narrativa foi efetuada com recurso à base de dados PubMed. Foi efetuada uma seleção utilizando as seguintes palavras-chave: “Endocarditis”, “Prophylaxis”, “Current evidence”,
“Clinical practice”. A seleção dos estudos foi feita pelos autores.

Resultados/Discussão: A incidência da endocardite é baixa (entre os 3 a 10 casos por 100 000) e variável entre os países desenvolvidos e em desenvolvimento. Ao longo do tempo, assistiu-se a uma mudança da abordagem da profilaxia antibiótica no sentido mais restritivo, sobretudo à custa da modificação da definição de doente de alto risco. Atualmente, a dúvida quanto ao seu real benefício ainda persiste. Por um lado pela falta de estudos randomizados e controlados, por outro pela evidência de níveis de bacteriemia semelhantes entre procedimentos dentários invasivos e medidas de higiene oral triviais.

Conclusão: Conforme o consenso das recomendações atuais, uma dupla conjugação de critérios deve ser aplicada para se proceder à realização da profilaxia, a existência de um doente de alto risco que irá ser submetido a um procedimento também de alto risco.
