Maculopatia Induzida por Raio: Relato de um Caso Bilateral com Sequela Atrófica

Resumo

Introdução e Objetivos: A exposição a descargas elétricas atmosféricas pode causar lesões oculares permanentes, frequentemente com predomínio macular. Pretende-se descrever um caso de maculopatia induzida por raio, documentado por imagiologia multimodal, e discutir a sua apresentação clínica e estrutural.

Materiais e Métodos: Relato de caso clínico. O doente foi submetido a avaliação oftalmológica completa, incluindo melhor acuidade visual corrigida (MAVC), biomicroscopia, tonometria, fundoscopia e exames complementares de diagnóstico (retinografia a cores e tomografia de coerência óptica).

Resultados: Um homem de 38 anos, previamente saudável, apresentou-se por queixas de escotoma central bilateral e hipovisão. A MAVC era de 0,7 em ambos os olhos. Referia ter estado internado no Brasil, cinco anos antes, após ter sido atingido por um raio. O segmento anterior e a pressão intraocular eram normais. À observação fundoscópica, em ambos os olhos, observavam-se, lesões maculares de cerca de um diâmetro de disco, centradas na fóvea, de limites bem definidos, hiperpigmentadas, compatíveis com cicatriz macular bilateral. A tomografia de coerência ótica de domínio espectral (SD-OCT) macular evidenciou adelgaçamento foveal, desorganização das camadas da retina neurossensorial, fibrose subfoveal e cavitações periféricas bilaterais, configurando um buraco lamelar em ambos os olhos.

Discussão e Conclusão: A maculopatia por raio pode manifestar-se com cavitações parafoveais e alterações da retina externa/EPR, refletindo dano térmico e elétrico preferencial em tecidos ricos em melanina. Pode evoluir, após a fase aguda, com sequelas estruturais irreversíveis, caracterizadas por atrofia da retina e formação de buracos lamelares, podendo em alguns casos evoluir para buracos de espessura total, condicionando défice visual permanente. O OCT é crucial para o diagnóstico, seguimento e aconselhamento prognóstico. Este caso ilustra uma sequela macular tardia, após atingimento por raio com repercussão visual, reforçando a importância de uma anamnese cuidada em doentes com maculopatia atípica.
