Primary Nurse – study case of family assessment and intervention in an Integrated Continued Care Unit

Introduction: The concept of Reference Nursing is based on assigning a nurse responsible for a group of users, guaranteeing continuity and centrality of care. The ‘Reference Nurse’ project developed in an Integrated Continued Care Unit (UCCI) in the north of Portugal is based on the Dynamic Family Assessment and Intervention Model (MDAIF), which is dynamic and flexible, allowing its components to be mobilized at the various levels of the nurse's work.
Objective: To identify diagnoses and intervene in order to achieve gains in family health.
Methods: A case study of a nuclear family integrated into the ‘Reference Nurse’ project. Family assessment and intervention were carried out through interviews with all family members and the analysis of narratives through content. The ethical principles of clinical research were taken into account.
Results: The diagnosis that required intervention was ‘Dysfunctional Family Process Related to Non-Effective Family Coping,’ and the interventions ‘Promoting Effective Family Coping’ and ‘Supporting the Family Coping Process’ were implemented. Reframing, positive connotation, and family conferencing were mobilized as techniques to implement the interventions, resulting in health gains.
Conclusion: As a result of the interventions aimed at family members, there was a change in family coping that had an impact on the patient's recovery and allowed family roles and beliefs to be maintained. The results demonstrate the relevance of focussing on family health using the MDAIF in an ICU context.
