The growing demand for health services requires more efficient management models to reduce costs and improve the quality of services. To discuss the impact of digital transformation on healthcare systems in Brazil and Portugal and the application of the Lean Healthcare approach as a solution, this article adopts a narrative bibliographic methodology. The research selected case studies and pilot projects implemented in both countries, highlighting significant advances in Brazil, such as telemedicine, mobile technologies, artificial intelligence, electronic medical records, and digital modernization. Portugal adopted a more comprehensive approach, investing in digital transformation and technological innovation, especially telemedicine. The leading digital transformation tools used in the Lean Healthcare approach include Strategic Planning, Integrated care, information and communication technologies, and telemedicine. It is concluded that digital transformation has been effective in providing satisfactory and efficient service in both countries, but there are still challenges to be faced. The continuous implementation of strategies based on digital transformation and Lean Healthcare is essential to further improve healthcare systems and meet future challenges. Both countries are at an early stage of digital transformation, and it is necessary to continue moving forward to achieve even more positive results.
