Contributions to the transition and integration of nursing care in palliative care

Introduction: Health institutions must mobilize strategies for the integration of palliative care, ensuring patients receive a response at the community level with telephone information transmission upon discharge.
Objective: Develop a Continuous Quality Improvement Project, with the transmission of phone information upon discharge; Understand the impact of phone transmission on community nurses; Know the degree of project completion.
Methods: Exploratory, descriptive, retrospective study, with the development of a Continuous Quality Improvement Project in a palliative care service in 2022. The degree of compliance with process auditing was assessed. The impact of this phone call on decision-making was assessed through a survey sent by email to community nurses. A descriptive analysis of the data was carried out. Ethical principles were safeguarded.
Results: Of the total of 546 patients, 157 were discharged in 2022. Of these, 144 saw the phone transmission of documented information. In the second phase, research was carried out with the identified nurses. From a total of 97 nurses, 70 responses were received. Most of these are from Family Health Units. They report that the phone call helped them make decisions about managing their treatment regimen.
Conclusion: The centrality of palliative care for patients is inherent in the development of communication bridges between the different providers of this care. The study examines hospital care and community healthcare partnerships.
