The influence of perineal massage from 34 weeks of pregnancy on preventing perineal trauma

Abstract

Introduction: Perineal trauma during eutocic labor is common and can result in significant physical and psychological consequences. In this context, perineal massage has been studied as an alternative approach to enhance tissue elasticity and reduce the risk of perineal trauma. However, the evidence regarding its effectiveness remains inconsistent.

Objective: To synthesize scientific evidence on the influence of perineal massage performed from 34 weeks of gestation for the prevention of perineal trauma during eutocic labor.

Methods: Systematic literature review following the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) methodology. Scientific databases searched included PubMed, B-on, CINAHL Complete, and MEDLINE Complete, followed by study selection based on inclusion/exclusion criteria, covering the time frame from 2018 to 2024.

Results: The analysis of the evidence from four eligible studies demonstrated that pregnant women who performed prenatal perineal massage had a significantly lower incidence of episiotomies and perineal tears, particularly the risk of third- and fourth-degree perineal tears. Better wound healing and less perineal pain were evident in the prenatal perineal massage group.

Conclusion: Prenatal perineal massage is associated with a lower risk of severe perineal trauma and postpartum complications.
