Introduction: Agenesis of the Corpus Callosum (ACC) is one of the most frequent congenital brain malformations and, when accompanied by other malformations, may be associated with severe neurological disorders and comorbidities, requiring specific rehabilitation care focused on individual and caregiver empowerment, as well as symptom control and minimisation, promoting comfort. Within the context of the Integrated Continuing Care Team (ICCT), the Rehabilitation Nurse promoted, within the scope of her functions, the maintenance of the person's functionality, namely respiratory function, at home.
Objective: To describe the care process in the stabilisation of respiratory function and the results focused on rehabilitation care, identifying health gains throughout the process.
Methods: A single, longitudinal case study was developed, following the CAse REport (CARE) guidelines, which retrospectively describes the rehabilitation care provided to a person with complex ACC, integrated in the ICCT, in whom respiratory function stabilisation was achieved.
Results: During the intervention period, 75 home visits of rehabilitation nursing were carried out, with improvement recorded in nursing diagnoses related to the caregiver's knowledge of the therapeutic regimen, dyspnoea, expectoration, and airway clearance. A reduction in the number of hospital inpatient days and in the need for antibiotic therapy was also observed.
Conclusion: Improving respiratory function through personalised rehabilitation nursing intervention within the ICCT contributed to the person's well-being at home and reduced the use of hospital services.
