Instrumentos para la evaluación en víctimas prehospitalarias de accidentes cerebrovasculares: una revisión scoping

Resumen

Introducción: En el área prehospitalaria, los enfermeros experimentan transiciones salud/enfermedad en las víctimas que presentan signos y síntomas de Ictus, utilizando diversos instrumentos para evaluarlas. Posteriormente, activan la Vía Verde, promoviendo beneficios para la salud de las víctimas.

Objetivo: Mapear los instrumentos de evaluación de signos y síntomas en víctimas adultas de ictus en el ámbito prehospitalario.

Métodos: Se realizó una revisión scoping siguiendo el método propuesto por el Instituto Joanna Briggs, con la siguiente pregunta de investigación: ¿Cuáles son los instrumentos de valoración de signos y síntomas en víctimas de ictus utilizados por el personal de enfermería en el ámbito prehospitalario? La selección de los estudios, la extracción y la síntesis de los datos fueron realizadas por dos revisores independientes.

Resultados: Destacan los siguientes instrumentos de evaluación: Face Arm Speech Test (FAST), Los Angeles Prehospital Stroke Screen (LAPSS), Melbourne Ambulance Stroke Screen (MASS), Rapid Arterial Occlusion Evaluation (RACE), Los Angels Motor Score (LAMS), Cincinnati Pre-hospital Stroke Severity Score (CPSSS), Field Assessment Stroke Triage for Emergency Destination (FAST- ED), National Institute of Health Stroke Scale (NIHSS) e Postural Assessment Scale for Stroke Patients (PASS).

Conclusión:  Todas las escalas tienen ventajas e inconvenientes que se traducen en su especificidad y sensibilidad. No obstante, deberían realizarse estudios primarios en la materia para comparar las escalas existentes y definir la “más” recomendable, así como explorar la aplicación de las escalas en diferentes escenarios prehospitalarios.
