Benefits of blood product administration in major trauma victims in prehospital care: scoping review

Abstract

Introduction: Trauma is a global public health problem, with a high mortality rate and decreased quality of life. In victims of major trauma, hemorrhagic shock represents one of the main causes of death in prehospital settings, highlighting the importance of administering blood products.

Objective: To map the benefits of administering blood products to victims of major trauma in prehospital settings.

Methods: Scoping review according to the Joanna Briggs Institute methodology using the Preferred Reporting Items for Systematic Reviews and Meta-analyses extension for scoping review flowchart. The CINAHL, MEDLINE, PubMed, and LILACS databases were searched. Quantitative and qualitative studies, as well as other scoping reviews, were considered.

Results: The administration of blood products in prehospital settings in victims of major trauma is associated with increased survival, reduced mortality, and better hemodynamic stabilization; reduced time to first transfusion; effective control over the components of the lethal diamond (coagulopathy, acidosis, hypothermia, and hypocalcemia); reduced volume of crystalloid usage, as well as the optimization of hospital and transfusion resources.

Conclusion: The results support the integration of blood product administration into protocols for managing victims of major trauma in prehospital settings.
