Benefícios da administração de hemoderivados na vítima de trauma major em contexto extra-hospitalar: scoping review

Resumo

Introdução: O trauma é um problema de saúde pública à escala mundial, com elevada taxa de mortalidade e diminuição da qualidade de vida. Na vítima de trauma major, o choque hemorrágico representa uma das principais causas de morte em contexto extra-hospitalar, evidenciando a importância da administração de hemoderivados.

Objetivo: Mapear os benefícios da administração de hemoderivados na pessoa vítima de trauma major em contexto extra-hospitalar.

Métodos: Scoping review segundo a metodologia do Joanna Briggs Institute com recurso ao diagrama de fluxo Preferred Reporting Items for Systematic Reviews and Meta-analyses extension for scoping review. Foram pesquisadas as bases de dados CINAHL, MEDLINE, PubMed e LILACS. Consideraram-se estudos quantitativos, qualitativos bem como outras scoping reviews.

Resultados: A administração de hemoderivados em contexto extra-hospitalar em vítimas de trauma major está associada ao aumento da sobrevida, redução da mortalidade e estabilização hemodinâmica; à redução do tempo até à primeira transfusão; à atuação eficaz sobre os componentes do diamante letal (coagulopatia, acidose, hipotermia e hipocalcemia); à diminuição do volume de cristaloides administrados, bem como à otimização dos recursos hospitalares e transfusionais.

Conclusão: Os resultados sustentam a integração de administração de hemoderivados em protocolos de abordagem à vítima de trauma major em contexto extra-hospitalar.
