Cuidados paliativos a la persona con insuficiencia cardíaca en cuidados críticos: scoping review

Introducción: La insuficiencia cardíaca es una enfermedad crónica que afecta a miles de personas en todo el mundo. En algún momento de su enfermedad, estas personas necesitan cuidados críticos. Los cuidados paliativos son esenciales durante todo el curso de la enfermedad.
Objetivo: Mapear la evidencia sobre cuidados paliativos para personas con insuficiencia cardiaca en cuidados críticos.
Métodos: Se realizó una revisión exploratoria según la metodología propuesta por el Instituto Joanna Briggs. La búsqueda se realizó en las bases de datos electrónicas MEDLINE Complete (vía EBSCO); CINAHL (vía EBSCO); Pubmed, Web-of-science y Repositórios Científicos de Acesso Aberto de Portugal (RCAAP, vía b-on).
Resultados: Se incluyeron 10 fuentes de pruebas en esta revisión. Los resultados se agruparon en 2 categorías: cuidados paliativos en urgencias y cuidados paliativos en la unidad de cuidados intensivos. Se identificaron 8 subcategorías: identificación de la necesidad de CP; control de síntomas; comunicación, toma de decisiones y planificación anticipada de cuidados; manejo de dispositivos cardiacos implantables y asistencia ventricular; el proceso de morir; cuidados culturalmente sensibles; definición de pronóstico; principios bioéticos y consentimiento informado.
Conclusión: Los entornos de cuidados críticos son lugares a los que acuden las personas con insuficiencia cardíaca en busca de ayuda para su enfermedad. Hay intervenciones específicas para cada contexto que pueden aumentar la calidad de vida de las personas.
