Cuidados paliativos à pessoa com insuficiência cardíaca em cuidados críticos: scoping review

Introdução: A insuficiência cardíaca é uma doença crónica que afeta milhares de pessoas em todo o mundo. Estas pessoas, em alguma fase da sua doença, têm necessidade de cuidados críticos. A prestação de cuidados paliativos é essencial no decurso de toda a doença.
Objetivo: Mapear a evidência sobre os cuidados paliativos à pessoa com insuficiência cardíaca em cuidados críticos.
Métodos: Foi realizada uma scoping review, segundo a metodologia proposta pelo Joanna Briggs Institute. A pesquisa foi realizada nas bases de dados electrónicas MEDLINE Complete (via EBSCO); CINAHL (via EBSCO); Pubmed, Web-of-science e Repositórios Científicos de Acesso Aberto de Portugal (RCAAP, via b-on).
Resultados: Foram incluídos na presente revisão 10 fontes de evidência. Os resultados foram agregados em 2 categorias – cuidados paliativos no serviço de urgência e cuidados paliativos na unidade de cuidados intensivos. Foram identificadas 8 subcategorias: identificação de necessidade de CP; controle de sintomas; comunicação, tomada de decisão e planeamento antecipado de cuidados; Gestão de dispositivos cardíacos implantáveis e de assistência ventricular; processo de morte, cuidados culturalmente sensíveis; definição de prognóstico; princípios da bioética e Consentimento informado
Conclusão: Os contextos de prestação de cuidados críticos revelam-se como locais aos quais pessoas com insuficiência cardíaca recorrem no recurso da sua doença. Existem intervenções específicas para cada contexto, que podem aumentar a qualidade de vida das pessoas.
